Dear New Patient,

Welcome to Northwestern Center for Orthopedics!

I would like to take this opportunity to tell you more about myself and my experience in orthopedic surgery and hand surgery. I grew up in Baltimore, Maryland, and studied American Studies at Williams College. After spending two years in a research lab at Johns Hopkins University, I entered medical school and graduated with honors from the University of Maryland School of Medicine in 1982. I completed an internship and second year of general surgical training at Johns Hopkins Hospital. I completed my orthopedic surgery training three years later at the Johns Hopkins Hospital. I pursued fellowship training in hand surgery at St. Vincent Hospital/Indian University one year later and completed my training in 1988. I completed a short fellowship in shoulder surgery at the University of Western Ontario. I have received specialized training in microsurgery and internal fixation of hand fractures. I served in the United States Navy as a reserve officer for over 14 years. I was the Chief of Orthopedics at the Great Lakes Naval Hospital during Operation Desert Storm.

My practice is focused on general orthopedics in children and adults and the treatment of disorders of the hand and upper extremity. It brings me great pleasure to apply state-of-the-art, proven procedures to restore mobility and help patients return to an active lifestyle. I strive to provide you as much information as possible to ensure that you make an informed decision on your treatment options. I joined the practice now known as Northwestern Center for Orthopedics in 1991, and since then it has been my mission to provide excellent service and care. I will attempt to start with conservative measures, therapy and rehabilitation for your problem. We will progress from there as your symptoms necessitate.

In addition to my extensive medical commitments, I enjoy horseback riding, outdoor sports, and spending time with my wife and three children.

Please let me know if there is anything I can do to further improve your experience at Northwestern Center for Orthopedics. My goal is to provide you the best medical care available.

I am looking forward to meeting with you.

Sincerely,

Charles Carroll IV, M.D.

I do realize that the extra time we require before your office visit might be a little inconvenient; however, it ensures that we have your correct information to help us better manage your care.

Additionally, I apologize in advance in case you have to wait to see me. In some cases, patients require special attention or I may have calls to the hospital to manage surgical patients' care. Under such circumstances, the staff of Northwestern Center for Orthopedics and I will do our best to see you as soon as possible and make sure you receive individualized attention.
